Welcome to Relational Advantage, Inc. We value your confidence and look forward to meeting you! NEW CLIENT INFORMATION: New client forms are available below. Please print, complete and sign where appropriate and bring them to your first appointment. If you are unable to open or print the forms for any reason please contact our office at 520-219-8377 and we will be happy to mail them to you. OFFICE LOCATION: Our address is 7355 N. Oracle Road, Suite 106, Tucson, Arizona, 85704. We are in the Family Life Radio building on the west side of Oracle Road, just north of Ina Road. INSURANCE: Dr. Linaman is contracted exclusively with Blue Cross/Blue Shield of Arizona PPO. If this is your primary insurance we will submit claims on your behalf. We recommend you call the Member Services number on the back of your card prior to your appointment to determine if you are required to obtain pre-authorization for outpatient mental health services. If you have other insurance that may reimburse you for services, we can provide you with necessary documentation upon request. PAYMENT: Fees, co-payments or co-insurances are payable at the time of service. We accept cash (correct change, please), personal checks, MasterCard and Visa.
